CUPW CPC is not sending earnings statement to WSIB- in Maple.

Mgt 1) The Supervisors complete the EE’s evouchers on a daily basis.

2) WSIB contacts AccessHR and requests the earnings information via 2 forms

- The short term earnings information form

-The modified earnings information form

3) AccessHR completes the earnings information forms and sends them back

to WSIB. This can take a minimum of 7-10 business days.

4) WSIB pays the employee.

WSIB will only pay the employee once WSIB has made a decision on a

claim.

CUPW Annual inspection

Supv needs to update every month. If the inspection is done in following

month, the employee will lose a month.

CUPW Whether there is a requirement for new hires (POCR on probation) to stay

at an office for a period of 6 months before they are eligible to transfer or

move?

Mgt Article 29.02- probation is 6 months

Article 12.08 to be deemed “qualified” ee must have completed probation
